There are only three forms needed to establish an FMA Student Chapter/National Honor Society on your campus. All the forms you will need are in the "Application to Charter" packet. Simply complete the Application To Charter and Chapter Information Form and create your chapter's Constitution and By-laws (you can use the sample Constitution as a guide or simply fill in the blanks!) Once you have completed the application packet, return it to the FMA office for approval. Charter approval takes between one and two weeks. Once your Application is approved, you will receive the approved application, your certificates of Charter for your Chapter and your National Honor Society and a copy of your approved Constitution and By-Laws in the mail.
